Cisco VoIP New Line Request
 
 
This form is to request a new line in the Cisco VoIP system. Please submit separate forms for each account that needs to be created.   The DoIT VoIP team hopes to complete this request in a timely manner.

If new equipment is needed for this request, please visit shopuw.wisc.edu to purchase phones and accessories.  We are not able to process requests for phones if the equipment has not been purchased. Shop@UW+ sells all of the Cisco desk phones, speaker phones and Jabra headsets.
