UW-Madison Qualtrics requests to permanently delete a survey can only be initiated by the survey owner. If you are not the owner of the survey to be deleted please contact the DoIT Help Desk.

 

Survey Names and Survey_IDs for each survey to be permanently deleted.

 

IMPORTANT: We can not recover any survey or the associated data once the survey has been permanently deleted.

 

This form can only be submitted by a current UW-Madison Faculty, Staff or Student. If you are no longer eligible for the UW-Madison Qualtrics service you will need to contact DoIT Help Desk for assistance.
