Welcome to the declaration form for UW's Undergraduate Certificate in Global Health. Please read the following information carefully:
If you are a UW undergraduate student, have completed at least one of the Certificate’s core classes, and have a GPA of at least 2.0 (both cumulatively and in the most recent semester), then you are eligible to complete a declaration form online.
You can fill out this form at any time. However, you are not officially declared until certificate staff have reviewed your form, passed it to CALS administrative staff, and had those staff formally activate the certificate in your student center.
Certificate staff only review declarations twice a year: after October 15th and after March 15th. Anyone wanting to declare in the fall semester MUST complete this form by midnight on October 15th. Anyone wanting to declare in the spring semester OR the summer term MUST complete this form by midnight on March 15th.
This form is intended to complement rather than replace in-person advising. Many college students say at graduation that they wish they had gotten more advice about choosing classes, things to do outside the classroom, and what to do after college. Don’t let that be you!
This form requires you to tell us at least tentative plans for your completion of the certificate’s core courses, electives, and field experience. This may require that you first spend some time with our electives list and on our field experience page.
You have up to 48 hours to complete this form. If you close it at any time, a cookie is placed in your browser. If you use the same computer and the same browser within 48 hours, the next time you click the survey link the form will see the cookie and load your previous answers.
Except where noted, you must answer all of the questions in this form.
