Madison Camp 2016
Welcome to the online student application for the South Central WI AHEC Health Careers Summer Camp. The student application contains 24 items, including one essay. Because you may NOT "save as you go", you are encouraged to first work through the application offline. You may cut and paste your essay into the application from your word processing program. A complete PDF version of the application is available on the AHEC website (www.ahec.wisc.edu).
The two teacher reference forms are available online and must be completed by the due date and will be verified by the Camp Coordinator. Due Date is April 13, 2016. Go to www.ahec.wisc.edu for the online reference forms.

South Central Wisconsin Area Health Education Center (SCAHEC) is sponsoring a health careers camp for high school students residing in the SCAHEC region who are interested in pursuing a health career.
The SCAHEC region by county includes: Adams, Columbia, Dane, Dodge, Green, Green Lake, Iowa, Jefferson, Lafayette, Marquette, Rock, Sauk and Waushara.
Camp Location is in MadisonSunday, June 19- Thursday, June 23, 2016Camp arrival is 2-3 pm on Sunday. Camp graduation is at 2:30pm on Thursday
The Madison camp activities will be held throughout the greater Madison area. Students will be staying at the Edgewood College campus.
A $150.00 camp reservation fee is due upon notification of acceptance to the camp. (Financial assistance is available if needed.) Please note that all sections of the online application must be completed by the student (including a student personal statement). In addition, there are two teacher reference forms that must be completed by two of the student's high school teachers, one of whom must be a Math or Science teacher, the other and English or Fine Arts teacher. The teacher reference forms are available online, must be completed by the application due date and will be verified by the camp coordinator.
Due to limited space at the camp, the Application Review Committee will process only complete applications.

In order for your application to be competitively reviewed for camp acceptance, you will need to have two online reference forms completed by two teachers. The online forms can be found on the WI AHEC system website www.ahec.wisc.edu. One teacher reference must be from an English/Fine Arts teacher and the other from a Math/Science teacher.
Please list both of the teachers names that will be completing online references for you.
Check back with your teachers before the due date to confirm they completed the online teacher reference form.
Reminder, the online references must be completed by your teachers by the application due date, Wednesday, April 13, 2016.
In approximately 300 words, please explain why you should be selected to attend the Health Careers Camp. Include your interest in pursuing a health care profession, future goals, work experience and other activities or information you would like the Application Review Committee to know.
